Request For Services Landing Page Final What is the Request for Services form?

The Request for Services (RFS) form standardises requests for member services. The RFS form can be
used to request over a dozen services for active
members and one service for deferred members
reducing the need to use different forms.

Why have we introduced the RFS form?

We developed the RFS form after recognising a need to streamline the request for services process. This included reducing the number of different forms used by employers and shared service providers.

When will you be able to use the RFS form?

The RFS form will be available to use from Monday 4 December 2017 and it will be available to download on the Employer Forms page or via the box below.

Important: you will still be able to use previous versions of forms to request member services up to Monday 5 March 2018. However, after this date, we will only accept requests made for member services using the new RFS form.
