- Please note that the phone number for the scheme medical adviser, Capita Health Solutions, has changed to 02476 500700.
- We have updated the medical referral forms on our website to include the new number. We have added 07 to the new versions of the forms (e.g. Med 2/07) to ease identification.
- Please try to use the website version of the forms at all times as this will always be the latest version. If you do need to use locally saved forms please ensure that you update them from our website on a regular basis.
