Anyone in your organisation who manages:
Complete and return the form, enclosed in the covering email, to email@example.com. We will need a completed form for each person in your organisation who wishes to attend. We will allocate places on a first-come-first-served basis although we ask that you keep nominations to a maximum of three so that we can get a fair representation from all CSP organisations. Please complete all fields on the form as this is an opportunity for us to check that we hold your correct contact details.
We will acknowledge applications immediately, and will send agendas and directions to all delegates the beginning of April.
The cost of attending the conference is free. However, if you have been accepted as a delegate then fail to attend, your organisation will be charged a £150 cancellation fee. We will, however, accept a deputy to attend on your behalf if your circumstances change and you cannot attend on the day. For security reasons, you must give us details of the deputy before 21 April 2009. If we have not received notification before then, they will not be admitted to the conference.
