Action: To note changes to instructions for completing order forms IHR1 and IRH2 for referrals to the Scheme Medical Adviser.

To note changes to instructions for completion of section 3 of order forms IHR1 and IHR2.

To note revised text on pages 1 and 6 of order forms IHR1 and IHR2 that reflect the changes to instructions for completion of section 3 of the order forms.

Timing: The changes to instructions for completing order forms IHR1 and IRH2 come into effect from 1 November 2009. The changes to instructions for completion of section 3 of order forms IHR1 and IRH2 and revised text on pages 1 and 6 that reflect those changes can be implemented with immediate effect. A copy of forms IHR1 and IHR2 with revised text for pages 1 and 6 are attached for your information and use.

  1. Order forms IHR1 and IHR2 for referrals to the Scheme Medical Adviser must be used in place of old forms from 1 November 2009. Any new cases referred to the Scheme Medical Adviser on old forms from 1 December 2009 will be returned to the referring department with a request that new forms be completed and submitted.
  2. We have agreed to issue a revised instruction that the completion of section 3 of forms IHR1 and IHR2 is recommended, but not mandatory. This revised instruction can be implemented with immediate effect. It should be noted that this revision is made to cater for organisations that have alternative methods of presenting the information required to fulfil the criteria outlined in section 3 to avoid duplication of records and effort. Information that fulfils the criteria outlined in section 3 is still required to enable the Scheme Medical Adviser to reasonably assess a case.
  3. The text on page 6 of forms IHR1 and IHR2 has been revised to reflect the changes to instructions for completion of section 3 as noted above. A copy of forms IHR1 and IHR2 with revised text for pages 1 and 6 are attached for your information and use.
