In place of signatures, we need members to prove their identity and address when making certain requests. We require members or death benefit claimants to provide:

For example, if you provide a driving licence and HMRC Tax Notification as proof of identity, you must provide another form of identification for proof of address, such as a utility bill. All bank statements and utility bills must be no more than 3 months old.

  • Benefits entitlement letter
  • HMRC Tax Notification
  • Local authority tax bill for the current year
  • Utility bill (such as gas or landline phone bill) less than three months old and must not be a mobile phone bill

  • UK, EU/EEA mortgage statement
  • Current UK provisional driving licence
  • UK credit card statement
  • Tenancy agreement issued by a solicitor, housing association, local council or reputable letting agency

Returning a form by email:

If you return a form to us by email, you can email a scanned copy or a clear photo of the ID documents with the completed form. 

Returning a form by post:

If you print a form and return it to us by post, please include certified copies or clear photographs if the ID documents in the envelope.
